Below is the form that you will need to complete BEFORE you are included in a mission trip. This helps us gather all the information needed so the Mission Coordinator can make the necessary transportation and lodging plans for the specific mission trip. Please be sure to complete ALL fields. Once you have completed and submitted the form, you should receive an email copy within 24 hours of the information you entered. You will need to print that email, sign it and return all pages stapled together with a check (unless you pay online) as described at the end of the form. If you make your deposit payment online, this will indicate your agreement to the terms on the application. Note that your application to go on the mission trip will only be considered AFTER the deposit has been received.
